In an email sent out by HR to the employees on the AFS/L3 contract the deadline for accepting requests for copies of personnel information is 1600 on Thursday, September 28. No request received after this time to HR will be accepted. To request a copy of your 201 file information please see your Admin Specialist and have them request the information for you. No requests from ACTIVE employees will be taken by phone.
All requests MUST include the following information:
- Badge (full 8-digits if subcontractor)
- Work Week (including off days)
Employees will be able to receive copies of the following items:
- Licenses provided
- Certificates/Diplomas provided
- Personnel Move History report (August 2005 – present)
- MDS Work History report (A/C mechanics & TIs)
Once notified that copies are ready for pick-up, they will be available at building 903 between the hours of 0600 – 1630 daily and employees will be required to show their badge to receive copies. No copies will be given to any individual other than the employee that they belong to.
For employees on Short Term Disability, Long Term Disability, or Military Leave, requests will be accepted via phone or email to JoAnn Camarata (334-598-0407), Adrienne Smith (334-503-3737), and Penny Poole (334-503-3759) only. These are the ONLY employees that will receive copies mailed to them, and they will be mailed to the home address on record at the request of the employee.
