What information is needed in order to fill out a retirement application?
Below is a list of information that you will need if you are interested in retiring:
- Pensioner's (if married, spouse's) birth certificate or two items showing birthday. Example: school record, baptism record, military records etc.
- If married, spouse's birth certificate or two other items as listed above.
- If Divorced, Divorce Decree for Females.
- Spouse's Social Security Number.
- If married, marriage certificate and/or marriage license.
- Date and place married.
- Union Book Number.
- Children's name, address and social security number, whether or not they are no longer living at home. If no Children, anyone you wish.
- This Applies to Retiring Men Only: If you are not your wife's first spouse, a legal type form must be submitted to the Pension Office, along with your application showing proof of your wife's maiden name linking it to her last husband's name. (may be your wife's children's birth certificate by previous marriage, prior marriage license, divorce decree, death certificate.) This form must be submitted to the Pension Fund Office.
- Need gross income for each year of your best three (3) years. May use W-2's used for income tax purposes.
- If divorced, items 2, 3,4,and 5 do not apply. You will only need a copy of divorce decree.
